Menopause is a significant event that every women experience in their lifetime. Menopause is the natural part of aging. After menopause significant physiological changes occur or symptoms noticed which affect the normal functioning of women, are referred to as postmenopausal syndrome. These symptoms adversely affect the quality of life.
What is menopause?
Menopause is defined as the condition in which there is permanent cessation of menstrual cycle occur. Menopause is diagnosed when menstruation is absent for 12 months. Mostly menopause occurs between the age of 45 to 55years the average age among women is 51 years.
Premature menopause
If Menopause occurs before the age of 40 years is defined as premature menopause. This occurs due to primary ovarian failure where there is a depletion of ovarian follicles (that are primary source of estrogen). The majority of symptoms after menopause are due to lack of estrogen levels.
How Menopause affects health?
- Menopause affects almost every system of women as menopausal women faces vasomotor symptoms, urogenital atrophy, osteoporosis, cardiovascular disease, cancer, psychiatric symptoms, cognitive decline, and sexual problems.
- It is important to understand the risk factors associated with menopause, its clinical presentation, and the management of common menopausal symptoms for better health. The four main symptoms in post-menopausal syndrome –
- Hot flashes,
- Poor sleep,
- Genitourinary symptoms/sexual dysfunction
- Mood changes
These are the most commonly symptoms seen among menopausal women are referred to as postmenopausal syndrome.
Pathophysiology of post-menopausal syndrome
Postmenopausal symptoms are mainly occurring due to the decreased level of estrogen. Hot flashes, sexual dysfunction and vulvovaginal atrophy result from the complex changes that occur around menopause. At the level of the ovary, there is reduction of ovarian follicles, mainly the granulosa cells. There is increase in Follicle-stimulating hormone (FSH) and luteinizing hormone (LH) level due to lack of feedback inhibition, and ovarian estrogen, progesterone, and inhibin production cease. However, the production of androgen hormone continues in the remaining ovarian theca cells and adrenal gland, which gets converted to estrogen through peripheral aromatization.
Effect of menopause on other systems
Heart/Cardiovascular System
Estrogens help to regulate blood flow , as estrogens have a positive effect on the inner layer of artery wall. Due to this reason , decline in estrogen after menopause may increase the chances of in heart disease among post-menopausal women.
Bone/Skeletal System
Decrease in levels of estrogen directly affects the musculoskeletal system. Musculoskeletal symptoms include backache, fractures on minimal injury, osteoporosis. Women after menopause at high risk of developing osteoporosis. It is a condition in which bones become very weak and brittle.
Urinary System
Lower levels of estrogen cause the thinning of the urethra lining. Also, weakness of the pelvic muscles those are present around the urethra. Muscular weakness occurs due to aging or vaginal childbirth. Due to weakness in muscles there is increased risk of urinary tract infections, urinary incontinence (bladder leakage) and many other problems.
Sexual Dysfunction
Natural lubrication in the walls of the vagina is maintained by estrogen levels. After menopause low levels of estrogen causes the vaginal tissues to become thinner and more easily irritated during sex—or dryness of the vagina occur. Due to this there is an increased risk of urinary tract infections and genitourinary syndrome of menopause, also known as atrophic vaginitis or vaginal atrophy.
Metabolism
Reduced estrogen may lower your metabolic rate, due to which body start to store fat instead of burning it. Other factors like aging , lower physical activity may also contribute to lower metabolic rate
Hormone replacement therapy
- As Menopause is caused by a decrease in estrogen and progesterone, so hormone replacement therapy (HRT) is an option to treat the health issues. For many women this therapy is helpful to lower the health risks that increase during their postmenopausal years.
Benefits of hormone replacement therapy: Estrogen therapy (with or without progesterone) may relieve night sweats, hot flashes, osteoporosis and vaginal dryness
Risks of this therapy: Estrogen-only therapy may increases the risk of breast and uterine cancer, heart disease, stroke and deep vein thrombosis.
Combine use of estrogen and progesterone also increase the risk of many diseases like DVT, Breast cancer etc.
Dosage & Age
Most experts recommend start taking from the lowest dose HRT for the shortest duration. Risk increase with the age. Lower the age lesser will be the risk of this therapy.
Topical or Oral use
- It’s also important to use the right type of estrogen for you. In vaginal dryness or menopause-related urinary tract infections or issues, a topical use of estrogen that is applied directly to the vagina could be the best choice.
- But if varieties of symptoms are present related to body systems then oral estrogen should be preferred.
Bioidentical or Traditional Hormone Therapy
Traditional hormone therapy uses man made hormones that are derived from plant or hormones found in the urine of pregnant horses. This can be taken orally, via patch, or topically to the genital area.
